Weight Loss in Five Treatment Options in a Structured Behavioral Program

Objective: Overweight participants need to be able to choose weight-loss options in a behavioral program that meet their specific needs. The purpose of this study was to document the weight-loss outcomes of five treatment options in a structured, research-based behavioral program.
Method: All patients who were enrolled in three Health Management Resources (HMR®) centers between January 1, 2002 and March 31, 2005 and completed "core classes" (12 wks weight loss) were evaluated (62% of entering patients completed core). HMR offers a range of weight-loss treatment options, including medically supervised low and very low-calorie diets, moderately restricted diets (Healthy Solutions®), and phone-based programs (HMR at Home®). A long-term maintenance program is also offered. All options focus on the same structured lifestyle changes: increased physical activity (>2000 kcals/wk) and the use of meal replacements (>35 servings/wk). Vegetable and fruit intake (>35 servings/wk) is also part of Healthy Solutions and HMR at Home weight loss. Weekly attendance and phone call commitments add accountability in all treatment options.
Results: Medically supervised patients completing core classes enter at the highest BMI (42.5 kg/m2) and lose the most weight (60.9 lbs or 22.6% of initial body weight). The average entering BMI of patients in the other clinic-based options range from 30.5 to 35.6 kg/m2. Patients in these options also lose substantial amounts of wieght, ranging from an average of 35.0—41.4 lbs and representing 18.4%—19.7% of initial body weight. Participants in HMR at Home, the phone-based program, lose an average of 35.2 lbs or 15.9% of initial body weight.

Conclusion: All program options far exceed the standard 5%—10% of initial body weight as is recommended for the health benefits of weight loss. By having a range of options available, all of which are based on the same structured, research-based behavioral program and result in successful weight-loss outcomes, any patient can be offered treatment.
