Three Behaviors Which Prevented Weight Regain in Maintenance

Objective: Maintenance of weight loss is more challenging for most than losing weight. Identifying the behaviors related to weight loss maintenance and specifying the minimum levels of these behaviors can enable patients to set goals and improve outcomes. The purpose of this study was to assess the impact that continuing to practice research-based behaviors had on the prevention of weight regain. In this study, the three behaviors were: physical activity (PA), vegetable and fruit intake (VF), and use of meal replacements (MR).
Methods & Subjects: Data were analyzed from 165 consecutive patients who started The HMR® Program for Weight Management™ (HMR) between 1/01/05 and 12/31/06 in 3 different centers and completed 6 months of maintenance.
Program: HMR offers a range of weight-loss treatment options, including medically supervised low and very low-calorie diets, moderately restricted diets and phone-based programs as well as long-term maintenance. The program is based on measurable, evidence-based variables allowing for specific goals to be set and compliance to be tracked. All options for weight loss and maintenance focus on the same lifestyle changes: increased physical activity (≥ 2000 kcal/wk); use of meal replacements (≥ 35/wk in weight loss and 14/wk in maintenance); increased vegetable and fruit intake (≥ 35/wk) is included in some weight-loss options and in maintenance.

- On average, patients who stay for at least 6 months of maintenance are doing large amounts of each of these behaviors: 2695 kcals PA/wk, 48.5 VF/wk, and 29.6 MR/wk and are experiencing virtually no weight regain at 6 months (-.14 lbs).
- Averages, however, conceal important details in the data. When each patient's data was analyzed for each week in maintenance, two points are detected:
- Different patients achieve different levels of MR use and VF intake to prevent weight regain
- Most patients seem to require different levels of these behaviors at different times in order to manage their weight.
Conclusions: Although specifying target behaviors for goal-setting is necessary, patients seem to find their own levels by strategically using MRs and VFs as necessary for successful weight management. PA was also variable within and between patients, but less so.
