Enrollment in HMR coaching sessions
Attending a minimum of 10 classes during the first 12 weeks of enrollment
Providing your employee number to HMR for the purpose of notifying Virgin Pulse on behalf of your employer, for meeting the 10-week attendance requirement
Choose Well eligible spouses or partners of an employee should provide employee’s ID with “01” added to the end of the number.
